Years after a mysterious pandemic's outbreak, hardly any survivors remain in North America's quarantined wasteland. Hunger and the threat of man-eaters pale in comparison to the ferals - those infected and mutated by the virus. The ferals have an unquenchable need to hunt and kill, and a single bite rapidly transforms a human into a rabid beast.

Two survivors, the pregnant Megan and her brother David, depart a deserted colony in search of a safer haven. On their way, a massive pack attacks them, but they narrowly escape with the help of two hunters. Cassius and Abigail have roamed the wasteland since the Breakdown, and as they debate whether to take in more strays, they realize the siblings are peculiar and that the ferals will relentlessly pursue them.

The group heads to a riverside colony where Megan can safely give birth. None could have foreseen the dangers awaiting them on their journey - or that the ferals might not be the gravest peril.
